  综述功能性近红外光谱成像技术(fNIRS)在脑卒中后失语症(PSA)中的研究进展,检索2015年1月至2025年5月PubMed、Web of Science、中国知网和万方数据库中fNIRS与PSA相关文献,采用物理治疗数据库(PEDro)量表评估纳入研究质量,2名研究者独立筛选并提取数据。最终纳入14篇文献,来自中国、美国、日本和韩国,累积323例PSA患者。发表时间主要集中于2020年以后,纳入文献多为对照研究。fNIRS 主要用于研究 PSA 患者的脑血流动力学变化和功能重组。常用指标包括脑区血氧浓度激活水平、脑功能连接性及图论特征等。fNIRS研究发现PSA患者在执行语言任务时常出现左侧语言区激活减弱、右侧及非语言区激活增加的代偿模式;静息态功能连接分析显示急性期患者的全脑连接显著降低,右半球功能连接增强与语言恢复相关。干预性研究表明,重复经颅磁刺激(rTMS)、间歇性θ波脉冲刺激(iTBS)及头皮针刺等治疗可改善语言功能;同时,fNIRS监测到对应半球语言区激活模式及网络连接的动态变化。fNIRS作为安全、无创的功能成像技术,为揭示PSA的脑损伤机制和功能重组规律提供了新视角;结合神经调控技术及传统针刺的研究不断涌现,提示fNIRS在指导个体化康复路径设计中具有潜在应用价值,未来需要更多高质量研究验证和完善这一技术在PSA康复中的应用。

    To systematically review the application of functional near-infrared spectroscopy (fNIRS) in post-stroke aphasia (PSA) rehabilitation research, relevant literature published from January 2015 to May 2025 was retrieved from PubMed, Web of Science, CNKI, and Wanfang databases. Two reviewers independently screened and extracted data. Study quality was assessed using the physiotherapy evidence database (PEDro) scale. A total of 14 studies involving 323 PSA patients were included, originating from China, the United States, Japan, and South Korea. Most were published after 2020 and employed controlled designs. fNIRS was primarily used to investigate cerebral hemodynamic changes and functional reorganization in PSA patients. Common indicators included regional brain activation (e.g., HBO concentration), functional connectivity, and graph-theoretical measures. Studies showed that PSA patients exhibit abnormal activation patterns during language tasks, such as decreased activation in left-hemisphere language regions and compensatory activation in contralateral or non-language regions. Resting-state studies reported reduced global and network-specific connectivity, with partial recovery in the acute phase. Intervention studies using repetitive transcranial magnetic stimulation (rTMS), intermittent theta-burst stimulation (iTBS), or scalp acupuncture demonstrated improvements in language function, paralleled by accompanied by fNIRS-detected dynamic changes in activation patterns and network connectivity within language-related hemispheres. fNIRS is a safe, non-invasive imaging technique with strong potential for revealing the neural mechanisms and recovery patterns of PSA. Its emerging integration with neuromodulation techniques and traditional acupuncture suggests potential utility in guiding personalized rehabilitation. Further high-quality clinical trials are warranted to validate and expand its clinical applications in PSA recovery.
