| Objective: To quantitatively evaluate and comparatively analyze the erector spinae, multifidus, psoas major, and quadratus lumborum muscles in the lumbar region of patients with unilateral lumbar disc herniation using shear wave elastography, and to explore the clinical application value of shear wave elastography in the diagnosis of lumbar disc herniation. Methods: All 30 patients with unilateral lumbar disc herniation meeting the criteria were recruited at Beijing Bo’ai Hospital. Shear wave elastography was used to examine the erector spinae, multifidus, psoas major, and quadratus lumborum muscles on both sides of the waist of the subjects in the prone position, and the shear wave Young’s modulus of the erector spinae, multifidus, psoas major, and quadratus lumborum muscles on both sides were obtained. The correlation between the shear wave Young’s modulus of the affected side muscles in patients with unilateral lumbar disc herniation and the degree of low back pain [(visual analogue scale, VAS) score] and [(Oswestry disability index, ODI) score] were analyzed. Results: In patients with unilateral lumbar disc herniation, the Young’s modulus of the multifidus and erector spinae muscles on the affected side was significantly decreased (P<0.01), and was negatively correlated with VAS and ODI scores (P<0.01); the Young’s modulus of the quadratus lumborum muscle on the affected side was significantly increased (P<0.01), and was positively correlated with VAS and ODI scores (P<0.01); while there was no significant difference in the Young’s modulus of the psoas major muscle between the two sides, and it was not correlated with clinical scores. Conclusion: The shear wave Young’s modulus of the erector spinae, psoas major and quadratus lumborum muscles in patients with lumbar intervertebral disc protrusion is greater than that of normal individuals. The shear wave elastography technique can effectively and objectively evaluate the hardness of the erector spinae, psoas major and quadratus lumborum muscles, providing an effective basis for the clinical diagnosis of lumbar disc protrusion. |
