EN Abstract:
  Objective: To describe the characteristics of full-dimensional attention function changes in patients with early-to-mid stage Parkinson’s disease (PD), explore classification based on attention function, and compare the differences in cognitive and motor functions among attention function subtypes. Methods: This was a cross-sectional study. A total of 112 patients with early-to-mid stage primary PD and 30 normal controls matched for sex, age, and years of education were enrolled. The digit span test, trail making test, stroop color-word test, and paced auditory serial addition test (PASAT) were used to assess each attention dimension. The mini-mental state examination and montreal cognitive assessment were used for global cognitive function, and the movement disorder society unified Parkinson’s disease rating scale part Ⅲ was used to evaluate motor function. We analyzed the characteristics of atte-ntion function changes in each dimension, performed exploratory cluster analysis to classify patients’ attention function, and compared the differences in global cognition and motor function among subtypes. Results: There were 41 patients in the PD with cognitive impairment group, who showed significant differences from the PD with normal cognition group and the normal control group in all attention dimension tests (P<0.05). In the 71 patients with PD and normal cognition, the errors in the trail making test and the interference amount of Stroop time consumption were significantly different from those of the normal control group (P<0.05). Exploratory cluster analysis suggested that PD patients might be classified into three types: high-transfer-high-allocation type, high-maintenance-balanced type, and low-transfer-low-selection type. Patients in the low-transfer-low-selection type had significantly lower global cognitive function and motor function than the other subtypes (P<0.05). Conclusion: Patients with early-to-mid stage PD show decline in attention selection and transfer functions, and other attention dimensions also decline as global cognitive function declines. The study observed that patients in the low-transfer-low-selection type had significantly lower global cognitive and motor functions than the other subtypes, providing preliminary evidence and a hypothesis worth testing for targeted assessment and treatment in future clinical work.
